This study aims to analyze the implementation of the Clean and Healthy Living Behavior (PHBS) policy at Sahabat Yatim Jagakarsa, South Jakarta City, and identify factors supporting and hindering its implementation. This research employed a qualitative descriptive approach involving managers, caregivers, and children under care as informants. Data were collected through in-depth interviews, limited participatory observation, and documentation analysis, while data validity was examined through source triangulation. The findings indicate that PHBS implementation has been carried out through daily hygiene routines, caregiver supervision, and children’s participation. However, the implementation process remains informal and has not been fully standardized. Communication is mainly conducted verbally without structured educational media, resources are available but require strengthening through caregiver training, and institutional commitment remains highly dependent on individual actors. The absence of written SOPs and specific PHBS evaluation mechanisms represents a significant institutional challenge. This study highlights the importance of strengthening organizational capacity through formal procedures, continuous education, and systematic monitoring to ensure sustainable PHBS implementation in child social welfare institutions.
